Comprehensive RN-guided planning and support -ideal for families navigating a new diagnosis or care transition. 

Includes:

• Initial consultation (Up to 75 minutes)

• Personalized written care plan tailored to your goals

• Tailored resource guide with trusted options and referrals

• Assistance creating a provider question list

• 3 hours of follow-up support within 30 day period

For families who need steady, trusted support each month. 

Includes up to 4 hours of RN-guided services per month, used flexibly to meet your needs. Time can be used tor:

• Monthly check-ins (Zoom, phone, or in-person)

• Care plan updates

• Appointment coordination 

• Reviewing documents or test results

• Researching care options or services

• Liaising with providers or community resources

If more time is needed, additional hours may be added at a reduced rate within the same 30-day period.

• Reviewing Explanation of Benefits (EOBs) and insurance statements

• Identifying errors or potential appeals

• Assisting with claim forms, supplemental  insurance paperwork, or reimbursements
