Consultation Services

A consultation (via in-person, phone, or virtual) to assess your situation and help you move forward with clear next steps.

30-Minute Consultation - $65
A focused, single-topic consultation designed to give you guidance and clarity on a specific question or concern.
Best for:

  • Understanding a diagnosis or recommendation

  • Receiving guidance in an urgent or time sensitive situation
     

60-minute Consultation - $125
A conversation designed to explore your concerns, review your situation in-depth, and begin developing a clear plan.

Best for:

  • Multiple concerns or complex situations

  • Navigating healthcare systems, providers, or care transitions

  • Identifying next steps and resources, including: medical, insurance, living needs, & more!

Care Navigation & Healthcare Advocacy Services - $125/hour | Billed in 15-min increments

After your consultation, all ongoing support is provided through personalized advocacy and navigation services tailored to your needs. Our care navigation and healthcare advocacy services provide personalized support for adults, seniors, family caregivers, and individuals facing complex healthcare decisions. 


This is where we do the legwork for you.

Services may include:

  • Coordinating care between providers and specialists

  • Scheduling appointments and preparing questions

  • Reviewing medical records and treatment plans

  • Insurance navigation, billing support, and claims assistance

  • Researching options for care, treatment, or second opinions

  • Hospital transition support, including hospital-to-home, rehabilitation, assisted living, and long-term care transitions.

  • Connecting you with trusted resources (legal, financial, housing, home care, hospice, and more)

  • Ongoing communication and updates for family caregivers

  • Creating a personalized care plan tailored to your needs

Payment & Service Information

  • Consultations

    • ​Payment is due at the time of service for all scheduled consultations.

    • Consultations may be conducted in person, by phone, or virtually.

    • Following your consultation, you'll receive personalized recommendations and next step guidance tailored to your situation. 

  • Care Navigation & Healthcare Advocacy Services 

    • A credit card on file is required for all ongoing services.

    • Services are billed in 15 minute increments. 

    • Clients receive a detailed summary of services provided and time spent. 

    • Ongoing support is personalized to your needs and may include care coordination, healthcare advocacy, caregiver support, resource navigation, and care planning. 

  • Financial Assistance

    • Thanks to the generosity of donors, grants, and community partners, financial assistance may be available for individuals and families.

    • Reduced-cost or no-cost services may be offered when funding is available. 

    • Please contact us to discuss available options.  We never want cost to prevent someone from reaching out for help. 

    • Funding availability varies and cannot be guaranteed. Assistance is provided based on available funding and individual circumstances.
