A one-time, up to 75 minute session to understand your family’s needs. Includes: - Medical and support needs overview - Identification of care gaps and priorities - Action steps and recommendations (In-person or virtually)
02.Hourly Advocacy & Navigation Support
Perfect for as-needed help. Services may include: - Reviewing and explaining medical reports or insurance statements - Coordinating care and scheduling appointments - Researching care facilities, providers, or equipment - Participating in care planning calls or family meetings
03.Care Planning Package
Ideal for families navigating a new diagnosis or care transition. (total time value is 4 hours and 15 minutes) Includes: - Initial consultation (Up to 75 minutes) - RN-reviewed personalized care plan - Resource guide tailored to your situation - Additional 3 hours of follow-up support during 30 day period - Assist in creating list of questions for your provider All time spent on support activities counts toward the 3-hour total.
04.Ongoing Monthly Support (Retainer Plan)
For families who need steady, trusted support. Includes up to 4 hours of RN-guided services per month, used flexibly to meet your needs. Time can be used for: - Monthly check-ins (Zoom, phone, or in-person) - Care plan updates - Appointment coordination - Reviewing documents or test results - Researching care options or services - Liaising with providers or community resources Additional hours available at a reduced rate of $100/hour. Unused hours do not roll over.
05.Insurance Review & Assistance
For individuals and families needing help understanding and managing healthcare coverage. Includes: - Reviewing Explanation of Benefits (EOBs) and insurance statements - Identifying errors or potential appeals - Assisting with claim forms, AFLAC/cancer insurance paperwork, or reimbursements - Liaising with insurance companies to resolve billing issues
